The Wellness Navigator – Family Plan provides personalized support to help families navigate their healthcare experience at Sound Clinic. Your Wellness Navigator serves as a resource for technology assistance, patient portal support, and coordination of services to make managing healthcare easier for your entire family.

The parent or primary purchaser will receive a phone call from a Sound Clinic team member to identify the family members included in the plan and collect their names and dates of birth to complete registration.

Wellness Navigator Services Include:

• Technology assistance with the patient portal and virtual care platforms.
• Help navigating online patient resources and healthcare tools.
• Appointment scheduling assistance.
• Assistance coordinating lab draws.
• Support with supplement and specialty test purchases.
• Coordination with practitioners regarding family members’ care needs and available services.
• Assistance coordinating up to 5 OMT visits, when applicable.
